Your Medicines Specialists
CPC consultants are a small group of accredited pharmacists who specialise in delivering medication and policy review services to those who require medication-related support. We have extensive experience in the disability sector, hospital and research. Our passion lies in supporting people who may be at a higher risk due to their medicines and setting, and we work with organisations, facilities and hospitals alike to improve quality assurance practices and safeguarding of medicines.

OUR SERVICES

MEDICATION REVIEW SERVICES
Our job is to help people understand and manage their medicines in the best way possible. Pharmacists are embedded in healthcare teams within hospitals, nursing homes, and can also be found working individually with patients in the community. By talking to people about their medications, pharmacists are often able to find solutions to increase health outcomes and reduce the stress medications can sometimes have on daily life.
TRANSITIONS OF CARE
Transitions of care, whether it is being released from hospital back home, or going from home into respite care, can be a risky time for managing medicines. Our role in helping these transitions include ensuring that all health care providers, carers, and support providers are kept in the loop with regard to tracking changes made to medicines. Transitions are an opportunity to evaluate a persons current health goals and optimise medicine use.


CONSULTING SERVICES
There is a growing demand for support providers in aged care and disability settings to ensure the protection and meet the health needs of their customers. Robust medicine policies embedded within a pro-active and evaluative service framework have proven benefits to reduce risk to the business and uphold the safety of customers. Our consultants work with organisations to provide advice on the best solutions for safeguarding customers, staff and operations
MEET THE TEAM

Hayley is an accredited consultant pharmacist and Diabetes Educator, and has more than 14 years community pharmacy experience. She currently has an academic position at the University of Newcastle, teaching into the Bachelor of Pharmacy and researching into Quality Use of Medicines
Dr Hayley Croft

Chelsea is currently undertaking her PhD in medicines use by people with a disability. She is an accredited consultant and community pharmacist, and teaches into the Bachelor of Pharmacy program at Newcastle University. Chelsea also currently sits as the NSW President of the Pharmaceutical Society of Australia
